What weight loss medications does Prema offer?
Prema offers Compounded GLP-1 Injections (with the same active compound as Wegovy® and Ozempic®) and Compounded GLP-1 Injections (with the same active compound as Mounjaro® and Zepbound®). We also coordinate prescriptions for the FDA-approved branded medications Wegovy®, Ozempic®, Mounjaro®, and Zepbound® when clinically appropriate. Prescriptions are issued only after a licensed provider consultation and if clinically appropriate.
How does the Prema GLP-1 membership work?
You start with a free consultation with a licensed Prema provider who reviews your medical history, current medications, and goals to determine whether you’re a candidate. If you are, your provider selects the appropriate compounded GLP-1 program and starting tier. Pricing starts at $149/month. The medication, weekly or monthly check-ins, and physician supervision are all included.
What's the difference between compounded and brand-name GLP-1?
Wegovy®, Ozempic®, Mounjaro®, and Zepbound® are FDA-approved, brand-name medications manufactured by Novo Nordisk and Eli Lilly. Compounded medications are prepared by a licensed compounding pharmacy on a patient-specific basis and may contain additional ingredients (such as B6) not found in branded versions. Compounded products are not FDA-approved and are not equivalent to or substitutes for the FDA-approved branded medications.
Who is not a candidate for GLP-1 medications?
Patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2, a history of pancreatitis, certain GI conditions, severe gallbladder disease, type 1 diabetes, or who are pregnant or breastfeeding are typically not candidates. What's the minimum commitment?
Three months, then month-to-month. Most weight loss programs lock you in for 6 to 12 months. After three months, cancel with 30 days’ notice. Freeze your membership up to 60 days per year.
Do I have to come into the clinic every week?
No. Three options: Full In-Person (weekly visits in Beachwood), Hybrid (mix in-person and virtual), or Full Virtual (telehealth from home with the same provider). Your choice from your first visit forward.
Are the consultations really free?
Yes. The consultation is free, and you’re under no obligation. A consultation does not guarantee a prescription. Medications are prescribed only after a licensed provider evaluation and if clinically appropriate.
Do you take insurance?
Aesthetic and weight loss services are not billed to insurance. We accept AfterPay, Cherry, Klarna, CareCredit, all major cards, and HSA/FSA. FSA/HSA eligibility varies by plan.
Where is Prema, and do you only offer telehealth?
Prema is at 3690 Orange Pl, Suite #150, Beachwood, OH 44122. We are an in-person medical practice first. Patient care is delivered in-person, hybrid, or virtually depending on your preference.

- FDA Boxed Warning · Thyroid C-Cell Tumors
GLP-1 receptor agonists, including Semaglutide and Tirzepatide, carry an FDA boxed warning regarding the risk of thyroid C-cell tumors, including medullary thyroid carcinoma (MTC). In animal studies these medications caused thyroid tumors. It is unknown whether this risk applies to humans.

- A personal or family history of medullary thyroid carcinoma (MTC)
- Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
- A known allergy to Semaglutide, Tirzepatide, or any GLP-1 medication
