What a First Appointment at Prema Actually Looks Like

From intake and labs to the first injection. A walk-through of how a new patient's first appointment actually flows, written by our care team.

Most patients who book a GLP-1 consultation at Prema have already done their reading. They’ve heard about Wegovy®, Ozempic®, Mounjaro®, and Zepbound®. They know compounded options exist. What they want from us is a real evaluation by a real clinician, in person, in Beachwood. That’s the visit.

Before you arrive

You’ll fill out a digital intake the day you book. Medical history, current medications, weight history, goals, and a short consent. We use this to flag anything that needs labs or a deeper conversation before we talk about a prescription.

What to bring

  • Your insurance card, if you’d like us to check coverage on FDA-approved branded options.
  • A list of current medications and supplements.
  • Recent labs, if you have them. We can also order what we need.

The clinician evaluation

This is the appointment. A licensed Ohio physician or our supervised clinical staff sit down with you, review your history, and have a real conversation about whether GLP-1 medication is appropriate for your situation. We talk about your goals, your timeline, and the trade-offs between compounded GLP-1 and FDA-approved branded options. For patients exploring physician-supervised weight loss for the first time, this conversation is where the real work begins.

If a GLP-1 isn’t the right fit for you today, we’ll say so. The evaluation isn’t a sales call.

Labs and screening

If labs aren’t current, we’ll either draw them in clinic or send you to a partner lab. The standard panel screens for the things you’d expect: thyroid markers, A1C, lipid panel, kidney function. The goal is a complete picture, not paperwork for paperwork’s sake.

The prescription, if appropriate

If a GLP-1 is clinically appropriate, we’ll discuss the two membership tiers and which one matches your situation. As a dedicated GLP-1 weight loss clinic, we include medication, visits, dose adjustments, and follow-up in the membership — no hidden add-ons. We dose conservatively at the start, follow up frequently in the first month, and adjust based on how you’re tolerating it.

Patients who have previously looked into an Ozempic® consultation or a Wegovy® consultation elsewhere are welcome to bring those records; we’ll review them as part of your intake and factor them into the clinical conversation.

Important safety information

GLP-1 receptor agonists carry an FDA boxed warning regarding the risk of thyroid C-cell tumors. Treatment is not appropriate for every patient. Individual results vary. The follow-up

This is what we think we do better than telehealth-only options. You’ll have a follow-up scheduled before you leave the first appointment. In-person, virtual, or hybrid, your choice. We see you back.
